Background

Form SSA-1128 (Representative Involved) attached to the cover of a claim file identifies a claim as involving a representative and requiring special handling. Therefore, the hearing office (HO) or Appeals Council staff must complete and affix an SSA-1128 to the claim file in all cases in which a claimant has appointed a representative.

Explanation of Content and Changes

We clarified I-1-111 to indicate that the HO or Appeals Council staff must affix form SSA-1128 to the cover of a claim file when a claimant has appointed a representative.

We made minor grammatical and punctuation changes in I-1-120.
