Action Note

In HALLEX I-5-437 V. B. 2. a., third bullet, change the code for the Clarksburg DDS from 510 to 511.

Background

On February 28, 1994, OHA published HALLEX Temporary Instruction (TI) 5-437 concerning implementation of the Boring v. Sullivan class action. Pertinent to screening for class membership, which is based in part on receiving an adjudication from the Clarksburg DDS, the TI instructed screeners to check for code 510, the code for the Clarksburg DDS, in item 2. of the SSA-831-U5.

Explanation of Content and Changes

The correct code for the Clarksburg DDS is 511. Code 510 is for the Charleston DDS. This “Action Note” is being issued to avoid incorrect screening for class membership, i.e., screen-out on the basis that the claimant did not receive an adjudication from the Clarksburg DDS.
