Information for RETAIN Phase 2 Participants
Welcome to the webpage for participants in the Retaining Employment and Talent After Injury/Illness Network (RETAIN). This is a survey for the workers who enrolled in one of the following RETAIN programs:
We hired a company, Mathematica to help with this survey. Mathematica will send you letters and call you two months after you enrolled in RETAIN to take the survey. Mathematica will follow up a second time about a year after you enrolled in RETAIN. Your participation will benefit not only yourself, but also others like you. Your response makes a difference!
If you have received a letter, or call, from Mathematica, please call Mathematica toll-free at 833-937-3178 and do one of these things:
- Ask for your username and password to complete the survey online. Mathematica will send you a $25 gift card as a thank you for completing the survey.
- Take the survey. You can also complete the survey over the phone. Mathematica will send you a $25 gift card as a thank you for completing the survey.
- Schedule a time to take the survey later and someone will call you back when you are ready.
- Let us know that you are not interested in participating so we can update our records.

How do I take the survey?
You can complete the survey over the phone, online, or on paper. To complete the survey over the phone, please call Mathematica toll-free at 833-937-3178. Mathematica can also arrange for you to receive a paper copy of the survey, or provide you with your username and password to complete the survey online.
Do I get anything for taking the survey?
Yes! You may have received your $5 in the mail to notify you about the study. You will get a $25 gift card for completing the survey.
Will my information be kept private?
Yes. We will never share your information with people outside the Social Security Administration and Mathematica unless required by Federal law. You can skip any questions you do not want to answer. Taking part in the survey will not affect any benefits you or your family may receive.
