Please refer to your request letter when completing this step. Most of these values can be found on your request letter directly below a graphic image called a "2-D Barcode" (shown below). When entering values for the "SITE/Destination" fields, you can perform either of the following:
- Enter the 3-character site code
- Select a destination from the dropdown
Entering the value for one field will automatically fill-in the value for the other. Also, the remainder of the page and further steps will display only after you have entered or selected a valid destination. Once you have selected a destination, you will be able to enter the other values from the request letter under Step 2.
