Select the destination requesting office to receive your submission. Please refer to your request letter when completing this step. Most of these values can be found on your request letter directly below a graphic image called a "2-D Barcode" (shown below). When entering values for the "Site/Destination" fields, you can perform either of the following:
- Enter the 3-character site code
- Select a destination from the dropdown
Entering the value for one field will automatically fill-in the value for the other. Also, the remainder of the page and further steps will display only after you have entered or selected a valid destination. Once you have selected a destination, you will be able to enter the other values from the request letter under Step 2.
