Please refer to your request letter when completing this step. Most of these values can be found on your request letter directly below a graphic image called a "2-D Barcode".

Here is some further detailed information for specific fields:
RQID (Request ID)
- For DDS destinations, enter this value as it shows on your request letter.
- For OHA destinations, enter this value beginning with the first non-zero number from the request letter. For example, if the request letter shows RQID as 0000001102400, enter only 1102400.
- If the "RF" field exists under the barcode, it can be one of three values: P, D, or blank.
- If the "RF" field does NOT exist under the barcode or if there is NO barcode on your request letter at all, indicate this by selecting "No RF or No Barcode" for this field.
- If the "DR" field exists under the barcode, it can be one of three values: F, S, or blank.
- If the "DR" field does NOT exist under the barcode or if there is NO barcode on your request letter at all, indicate this by selecting "No DR or No Barcode" for this field.
- If the "CS" field exists under the barcode, it can be up to four characters in length or blank.
